It is essential for us to obtain feedback so we can inform the Big Lottery on how the GO FAR project is going.

Please note that Jigsaw GO FAR activities relates to:

•  Making the Most of Me Programme
• Communications Programme
• Feel Good Workshops
• Essential Volunteer Training
• GO FAR Shopping & Cookery
• Horse Riding
• Monday night Aerobics
• Gym sessions

To fill the forms in, please circle a number between 0 and 10 to represent your answer. Please do not circle the words (such as not at all or hugely)
